Patient Portal Change

We are now live with the FollowMyHealth Patient Portal!

    We made this change because the new FollowMyHealth portal provides a more user-friendly experience with more features for our patients. You will enjoy a more comprehensive view of your health with more ways to track your health and communicate with us.

    If you are a current Medfusion portal user, there are a few things you need to know:

  • All communication has been turned off from the Medfusion portal to our office.

  • Until the end of July, you will be able to access your information in Medfusion, but no new information will be updated and you will not be able to communicate with us from there.

  • Around July 23rd, you will receive an invitation to join FollowMyHealth. This will require you to create a new username and password, or you can log in using an existing Facebook, Google, Yahoo, Microsoft, or Cerner username and password.

  • Once you join FollowMyHealth, your health information will immediately update from your medical record, and you will be able to securely request appointments, request prescription refills, and message with our office through FollowMyHealth.

  • Click here to continue to the Medfusion Patient Portal.

  • If you are not a current portal user but we have your email address on file, you will also receive an invitation to join FollowMyHealth around July 23rd. We hope you will join and take advantage of all the features FollowMyHealth has to offer!
